The Modern Warfare 4 campaign has 16 missions set across the Korean Peninsula, Paris, New York and Mumbai, according to Call of Duty's official blog. Digital pre-order owners got Campaign Early Access on October 16, 2026, and the full game launches on October 23, 2026. This guide lists every mission in order with its reward, plus the 18 trophies and achievements and what outlets report about difficulty and length.

Modern Warfare 4 Campaign Release Dates
The official Campaign Early Access post, dated October 16, 2026, says Campaign Early Access is live and that "Modern Warfare 4 launches in full on October 23." The official PC blog gives the same two dates.

The order below matches GameSpot, Game Rant and Dexerto. The rewards follow the order and wording on callofduty.com. The official post says all rewards become available in Multiplayer and DMZ when the full game launches.

Two naming differences between sources:
- Mission 11. GameSpot calls it Defection. Game Rant and Dexerto call it Deflection. We use GameSpot's spelling until an official list appears.
- Mission 5 reward. callofduty.com names the loading screen "Ready Line". GameSpot and Game Rant write "Ready Live".
Mission Objectives
We have not found a reputable source that describes the objective of every mission, so this guide does not list one per mission. What outlets do report:
- Stranded is the one open-world mission, with optional exploration, per GameSpot. In its campaign review, GameSpot says looting supplies there earned Park a suppressed sniper rifle that helped later.
- Game Rant says you can replay completed missions, which matters for the mission-specific trophies below.
Modern Warfare 4 Trophies and Achievements
Modern Warfare 4 has 18 trophies and achievements, all tied to the campaign. Game Rant published the full list. TechWiser and All Things How report 8 Gold and 10 Silver, worth 1,000 Gamerscore on Xbox. There is no Platinum on PS5. Game Rant says that is because the game runs through the Call of Duty HQ app.
Steam's achievement page for the game had no stats yet when we checked, since the Steam version is listed for October 22. We could not open the PlayStation or Xbox lists directly, so the table cites the outlets above.

Game Rant and All Things How spell the Entrenched trophy "Trench Boom". TechWiser spells it "Trench Broom". Tier and hidden status come from TechWiser and All Things How.
Collectibles
The trophy list names the collectibles that count:
- Notes. Noteworthy needs 50 different Notes. No Stone Unturned also needs every note in Following Orders.
- Contraband in Following Orders, for No Stone Unturned.
- Yellow Supply Crates in Stranded, for Time to Crate.
- Dog Tags in Escape, for Never Forgotten.
- Base Camp repairs, for Scavenger.
We have not found a reputable source for exact note locations or for any safe, lockbox or keypad codes, so this guide does not list them. Codes posted as player tips are not included.
Modern Warfare 4 Campaign Difficulty Options
The trophy list confirms Hardened, Veteran and Realism difficulties. Game Rant says Realism unlocks only after you finish the campaign once, and that finishing also opens a YOLO difficulty. We have not found an official description of each difficulty from Activision.

Our suggestion. If you plan to hunt trophies, play your first run on Hardened. That lets you try Safety's On and Revenge on the way, and any full clear also earns Hold the Line and opens Realism. If you only want the story, pick the setting you are comfortable with, since replaying missions later is an option.
How Long Is the Modern Warfare 4 Campaign?
Outlets give slightly different figures:
